The person has a documented terminal diagnosis with a life expectancy of six months or less if the illness follows its expected course. Patient should have been informed of diagnosis by the primary physician.
The primary physician has explained to the patient and/or the family that Mahogany Hospice Care will initiate no resuscitation measures.
A DNR (do not resuscitate) order does not have to be signed for admission.
The person is under the care of a physician who will continue to assume responsibility for medical care.
The person wishes to stay at home for as long as possible.
There is an identifiable, willing, capable caregiver in the home unless the patient, the family and the Hospice team agree to other appropriate arrangements.
The family needs support in caring for the person and agrees to Hospice care.
The person lives within the Mahogany Hospice Care service area.
